Aniket Chakrabarti, MD

Aniket Chakrabarti, MD, is a graduate of Kasturba Medical College, Manipal India. He did his residency in internal medicine at Carney Hospital in Boston, MA. Dr. Chakrabarti has been practicing since 1999. For the last 22 years, he has been founder and physician at Pembroke Primary Care, an independent Internal Medicine & Primary Care practice located on the south shore of Massachusetts in Marshfield. He specializes in primary care and geriatrics with an emphasis on preventative medicine. His interests include diabetes, obesity, end-of-life issues, palliative care, pain relief strategies, and teaching.

Dr. Chakrabarti has been an Adjunct clinical faculty and Assistant Professor at Tufts University School of Medicine for over two decades during his career. He is a preceptor for Tufts Medical School Medical students, Physician Assistant students, and Nurse Practitioner students from various colleges and universities. His passion for developing these practitioners of medicine is evident in the close relationships and communications he shares with his students.

While maintaining an active and busy practice and mentoring students, Dr. Chakrabarti also has extensive experience as a Medical Director of several Skilled Nursing Facilities, Assisted Living Facilities, Hospice Services, and Home Health Agencies. Dr. Chakrabarti is an active member of the American Medical Directors Association, Massachusetts Medical Society and Hospice & Palliative Care Federation. He sits on the Board of the Medical Executive Committee at Carney Hospital and maintains close relationships with the Medical Boards of several hospitals within the communities in which he practices.

Dr A.C. is also knowledgeable and interested in helping his patients with integrating alternative therapies in their care. This includes medical marijuana and CBD among others to help facilitate his patient’s care.

Dr Chakrabarti was a founding partner and Chief Medical Officer of Clinicore Health Solutions, LLC, a premier telemedicine and health solutions company based in Massachusetts back in 2007 which pioneered the use and implementation of telemedicine technology in nursing homes across Massachusetts. Finally, in his free time, he enjoys travelling, he is an avid photographer and loving husband and father of two beautiful daughters.
